NY Connects: Choices for Long Term Care
A joint initiative with United Way now operating

NY Connects is a toll-free phone number that county residents can call to quickly and readily learn about long-term care options (in Spanish) for seniors and for disabled children and adults. The number is (877) 914-4040.

NY Connects helps you to learn about the various long-term care choices for your specific situation. Armed with that information, you'll be able to make a better-informed decision on the best option for you or your loved one.

Calls are answered 24 hours a day, seven days a week by trained specialists and all calls are confidential. Help is provided in several languages, and TTY is available for the hearing-impaired.

Long-term care involves medical and non-medical services, and NY Connects provides a plethora of information relating to a wide range of topics, including Medicaid and Medicare, in-home and out-of-home care, transportation, health, recreation, advocacy, insurance and financial planning.

As you sort out the various options, you might decide that assisted-living housing best fills your needs. Or, you might realize that a combination of adult day-care and physical therapy would be a better fit.

Give NY Connects a ring, too, with questions about short-term care for yourself or a loved one.  If you seek rehabilitation or home care after an illness, just pick up the phone and call.

NY Connects: Westchester Choices for Long-Term Care is a joint, state-funded initiative of the Westchester County Departments of Senior Programs and Services and Social Services, the Westchester County Office for the Disabled and United Way of Westchester and Putnam.
